What We Need From You

We're ready to get started on your customized internal marketing package - but first we need a few things from you. Please provide the following materials as soon as possible:

 

1. Data Export

This is the first and most important step to get you started. If you haven't already scheduled a data extraction, please call us at 1-800-359-3937 for an appointment. Or if you prefer to send your data without assistance, please export your patient information into a Microsoft Excel spreadsheet or a tab/comma delimited file (using the field order listed below):

  •  Patient ID*
  •  Last name
  •  First name
  •  Address 1
  •  Address 2
  •  City
  •  State
  •  ZIP code
  •  Home phone
  •  Cell phone
  •  Work phone
  •  E-mail address
  •  Birth date
  •  Gender
  •  Date of Last Visit

* A required field that serves as the unique identifier of the patient (i.e. chart #, patient #).

If you choose not to schedule a data extraction, you may send your electronic file via e-mail. If you need assistance, your software provider's customer support should be able to help you with the download, and some may even provide instructions online.

 

2. Photos

Photo size (resolution) should be 300 DPI or higher, or you can send two original photos to be scanned. Acceptable file formats include JPEG, TIF or EPS.

Please no .zip or .exe files.

  •  Staff (group) photo

You may send your photos to us by mail or e-mail. Please include the dentist's name and the practice phone number with mailed items. If you would like us to return your photos, please inform your Patient Activator representative.

 

3. Logo Art (if applicable)

If you have a logo, please send it to us in .ai (Adobe Illustrator file), .eps (Encapsulated Post Script) or .fh (Macromedia Freehand file) format.

 

We are also able to accept JPEG and TIF files if they meet the minimum size requirement of 300 DPI. If you have questions about these formats, please feel free to contact us. If you do not have your logo in one of the formats listed above, please contact your logo designer - or you may select an alternate image from our Colors & Images page.

 

You may send your logos to us via e-mail.

 

4. Member Information Form

To help us customize your patient communications to fit the tone and image of your practice, please download and fill out our Member Information Form, indicating your preferred communication topics.
