This Web site is administered by Futuredontics, Inc.  As part of the normal operation of this site, we collect and disclose information about you. This Privacy Statement describes the information we collect about you and what may happen to that information.

 

Information We Collect and Our Use of That Information

In the course of using patientactivator.com and members.patientactivator.com, we automatically track certain information about those users. This non-personally identifiable information includes the URL that you just came from (whether this URL is on our site or not), which URL you go to next (whether this URL is on our site or not), what browser you are using, and your IP address. Most sites automatically collect this information.

 

We may also collect and store any information directly entered at members.patientactivator.com or give us in any other way, including personally identifiable information (which may include our member's name, address, telephone and cell numbers and e-mail address, as well as our member's patient data). Any personally identifiable information is used for the Patient Activator program or our internal business purposes only.  This personally identifiable information will never be sold or shared with any third party not associated with the program. We will gladly complete a HIPAA Business Associate agreement with member practices and/or potential member practices upon request.

 

In addition, we use cookies on certain pages of our site. Cookies are stored on your hard drive, not on our site. You are always free to decline our cookies if your browser permits, although in that case you may be required to re-enter certain information more frequently during a visit to our site.

Futuredontics, Inc. uses industry-standard efforts to safeguard the confidentiality of your personally identifiable information, such as firewalls and Secure Socket Layers. However, "perfect security" does not exist on the Internet.
